Penn Center for Community Health Workers: Making a Difference in Their Community

Penn Center for Community Health Workers has served more than 6,000 high-risk patients in Philadelphia, working to improve their health and reducing their hospitalizations by 30 percent. The secret? The center’s 60 full-time employees help patients with everything from housing to diet to medications. The community health workers who meet with patients and families share similar backgrounds and demographics. They “get” what patients are struggling with and can help them overcome challenges.
